After scheduling an appointment, you will receive a registration link from eDerm. If you do not receive this registration link please contact our office.
Log in with the patient’s last name and date of birth. If you didn’t receive the email, please check your Spam Folder. If you still can’t find the email or can’t log in, please call the office.
Please arrive 15-20 minutes prior to your appointment time.
New Patient Forms are found on our website home page under patient forms. We ask that you print and complete these forms and bring them with you at the time of your visit.
If you are a new patient, we ask that you arrive 15 minutes ahead of your appointment.

If you have any questions about our participation in your insurance plan, please call our office or your insurance company before scheduling an appointment. It is suggested that you reach out to your insurance company to make certain we are an in-network provider. Salida Dermatology is contracted with Integrated Dermatology and might be referenced with your insurance under that entity name.
If your plan requires a Referral to be seen by a Specialist, please obtain it from your Primary Doctor before your visit.
Our online scheduler allows patients to self-schedule up to 90 days into the future. If you are an established patient, you should see a “welcome back!” notice appear after validating your name and date of birth. If you do not see this even though you have been a patient of the practice, please call the office after scheduling your appointment to verify your information, before filling out the registration form which will be e-mailed to you.
**Please make sure to review our registration policies as we enforce a "No Show Fee" for appointments that are not canceled 24 hours in advance.
